Box 1127 Fuquay Varina, NC 27526 GPS COORDINATES: N: 35.63138 E: -79.01333 FACILITY CONTACT NAME AND PHONE NUMBER: Name: Wallace Woodall Telephone: (919) 577-1995 Email address: ecrww@yahoo.com Fax: (919) 577-1998 FACILITY CONTACT ADDRESS: Same as above PARTICIPANTS: Connie Wylie, Wallace Woodall STATUS OF PERMIT: Active – permit expires January 15, 2020 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: 1. .0838(e)(1) Each detention and treatment facility shall be designed, constructed, and maintained to prevent leaks or the flows of septage out of the facility into the seasonally high water table, onto the ground surface, or into any surface waters. While inspecting the area up top where the trucks unload into the storage tanks an area was observed to the right near the tanks where a grease spill had occurred. Upon inspection down below at the tank level it was noted that the grease had flowed down between the tank & the wall. Mr. Woodall stated that the spill may have occurred while maintenance work was performed. Instructed Mr. Woodall to clean up all of the spill area (up top and down below) completely and he stated that it would cleaned up. Also in the course of any maintenance work it is not permitted to throw waste contaminated debris/equip or actual septage onto the ground surface – it always should be contained & disposed of properly. The unloading area & storage tanks should be routinely monitored to ensure compliance. Any spills or overflows that may occur should be immediately cleaned up. The three above ground tanks appear structurally sound but are still tilted. 2. The access road is adequate and the required signs are posted. 3. No vectors or odors noted. 4. The approved setbacks are being met. 5. The required 5 foot cleared radius (free of excess vegetation or debris) is present around the tanks at this time. 6. Received a renewal application from Mr. Woodall (this permit expires January 15, 2020). Mr. Woodall stated that he wished for the new permit to just be a detention facility for the existing three tanks (not a dewatering facility as previously permitted but never constructed). Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
