STATUS OF PAST NOTED VIOLATIONS:
The lettering has been corrected on Vehicle tag#HK5530, Vehicle tag#YA121426, Vehicle tag#JL9165 and
Vehicle tag#JM8117. These four vehicles are now in compliance.
Vehicle tag#FD8434 and vehicle FL tag#882-MPP were not available for reinspection during this visit – still need
to verify compliance.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#JT4297, VIN1FDUF5GTXJEB44202, 750 gallons and vehicle tag#HA2029,
VIN1HTMMMML3HH710320, 1000 gallons. On both vehicles the tanks appeared structurally sound &
securely attached. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides
of both vehicles per the information on the firm application.
2. Vehicle tag#JM-8038 was previously inspected on 7/18 (and again during this visit) and was found to be in
compliance.
3. The firm permit was reissued by the Raleigh Central office on July 30, 2019 to reflect the firm name of “Blu
Site Solutions” as displayed on all the permitted vehicles.
4. Ensure that the renewal application submitted this winter for the 2020 firm permit lists the correct tag#s
& VINs for all vehicles to be covered under the firm permit for the Johnston County location. This office
shall be notified (within 10 days) of any new trucks that are added to this location so that an inspection
may be performed.
