Executive Summary The subject property consists of one parcel totaling approximately 0.145 acres located at 411 Woodvale Place in Charlotte, Mecklenburg County, North Carolina (Parcel ID No. 07107412). This property includes a single parcel and is currently undeveloped. The property location is illustrated on Figure 1. The adjacent and surrounding properties are primarily residential and Woodvale Place fronts the subject property to the north, beyond which are residential properties. The adjoining properties to the south, west and east are also residential properties. The subject property layout and adjoining properties are illustrated on Figure 2. In April 2018 during the demolition of the former onsite residence, one estimated 550-gallon fuel oil underground storage tank (UST) was removed from the central portion of the subject lot. The UST was removed from the ground by the demolition contractor. Prior to removal of the UST, 450-gallons of petroleum contact water (PCW) was removed from the tank and transported off -site for disposal. During the UST removal olfactory indications of hydrocarbon affected soil in the vicinity of the UST were detected. On May 4, 2018, PM was contracted to collect a soil sample from the hydrocarbon affected soils in the area of the former UST location. One soil sample (TP-1) was collected at the approximate depth of the former basin bottom. The soil sample was submitted for laboratory analysis of Total Petroleum Hydrocarbons — Gasoline Range Organics (TPH-GRO), Total Petroleum Hydrocarbons — Diesel Range Organics (TPH-DRO). Review of the soil analytical results indicated TPH-DRO at concentrations of TPH-DRO 3,680 milligrams per kilograms (mg/kg) exceeding the North Carolina Department of Environmental Quality (NCDEQ) reporting limit of 100 mg/kg. Based on the results of the May 2018 sampling results, the property owner contacted the NCDEQ notifying them of the release, which prompted a Notice of Regulatory Requirement (NORR) issued by NCDEQ dated July 6, 2018. The NORR issued a requirement for Initial Abatement Action (IAA) to be completed. Additionally, the NCDEQ assigned an incident number (40822) for the release. On August 7, 2018, PM performed the oversight of approximately 66.99 tons of hydrocarbon affected soils from the former UST basin. The excavated soil was transported to Environmental Soils, Inc. of Lattimore, North Carolina. Five subsurface confirmation soil samples (OE-1 through OE-5) were obtained from native material from the tank basin sidewalls and base following the soil excavation. Each of the confirmation soil samples were submitted for laboratory analysis of volatile organic compounds (VOCs), semi -volatile organic compounds (SVOCs), and Massachusetts Department of Environmental Protection Volatile Petroleum Hydrocarbons and Extractable Petroleum Hydrocarbons (MADEP VPH/EPH). A review of the analysis from the confirmation soil samples obtained from the former tank basin revealed EPH Aromatic fraction C-11-C22 at a concentration of 28.9 milligrams per kilograms (mg/kg) in sample OE-1 collected at the base of the excavation. The detected concentration of C-11-C22 is below the NCDEQ most stringent soil to groundwater Maximum Soil Contaminant Concentration (MSCC) of 31 mg/kg. No additional target analytes were detected in any of the confirmation samples exceeding the laboratory MRLs. PM Environmental, Inc. UST Closure Report In April, 2018 during demolition of the former onsite residence, one fuel oil UST was identified and removed from the central portion of the subject property. Prior to removal of the UST, 450- gallons of PCW was removed from the tank and transported off -site for disposal. During the UST removal olfactory indications of hydrocarbon affected soil in the vicinity of the UST were detected. The UST was estimated to have a capacity of 550-gallons, measured 3.5 feet in diameter and seven feet in length. Additional information pertaining to the date of installation, use, or contents of the UST is unknown. The location of the tank is illustrated on Figure 2. The overburden soil was removed with an excavator and stockpiled. The top of the tank was encountered approximately two -feet below the ground surface. Tank measurements were: • Diameter: 3.5 feet. • Length: 7.0 feet. • Capacity: 550 gallons. • Construction: Steel. Soil excavation and subsurface sampling are discussed in Section I. G. Initial Response and Abatement Action On August 7, 2018, PM performed the oversight during the excavation and disposal of approximately 66.99 tons of hydrocarbon affected soil from the former UST basin. H. Groundwater and Surface Water Investigation Groundwater was not encountered during this Initial Abatement investigation. Surface water was not addressed during this investigation. I. Excavation of Contaminated Soil Excavation and Sampling: On August 7', 2018, PM performed the oversight of Elite Environmental Services, Inc. (Elite), utilize an excavator to remove subsurface soil, including hydrocarbon affected soil, from the former UST basin. The final excavation dimensions of the basin, located near the central portion of the subject property, are as follows: PM Environmental, Inc. Page 7 Initial Abatement Action Report for the Siblings, LLC Residence Located at 411 Woodvale Place, Charlotte, North Carolina PM Environmental, Inc., Project No. 15-4305-0-0002; August 28, 2018 • Excavation Length (east -west): 14 feet. • Excavation Width (north -south): 16 feet. • Excavation Depth (bgs): 8 feet. Soil was excavated approximately 1.5 to 2.5 feet laterally and approximately 2.0 feet vertically from the former tank location, and directly loaded into dump trucks. The excavated soil consisted primarily of silt and clay fill with a hydrocarbon odor. Overburden soil which did not exhibit hydrocarbon odors or photoionization detector (PID) readings were stock piled onsite. Five soil samples were collected from the sidewalls and base of the soil excavation (OE-1 through OE-5). The soil sample locations are illustrated on Figure 3. Each of the soil samples were placed into laboratory containers, labeled, and transported to Prism Laboratories, Inc. of Charlotte, North Carolina (a North Carolina certified laboratory) under chain of custody procedures. The samples were analyzed for MADEP VPH / EPH, volatile organic compounds (VOCs) per EPA Method 8260, and semi -volatile organic compounds (SVOCs) per EPA Method 8270. Following the soil excavation the basin was left open pending the results of the confirmation samples. Approximately 66.99 tons of hydrocarbon affected soil were removed from the former UST basin. The excavated soil was transported to Environmental Soils, Inc. in Lattimore, North Carolina for disposal. Soil disposal manifests are included in Appendix A. Analvtical Results: Soil samples were obtained from the excavated basin sidewalls and basin floor, examined, and scanned with a photoionization detector (PID). The sample identifications, locations, and PID results were as follows: • OE-1, basin bottom, 8 ft. bgs, 0.0 ppm (PID). • OE-2, west side wall, 5 ft. bgs, 0.0 ppm (PID). • OE-3, south side wall, 5 ft. bgs, 0.0 ppm (PID). • OE-4, east side wall, 5 ft. bgs, 0.0 ppm (PID). • OE-5, north side wall, 5 ft. bgs, 0.0 ppm (PID). Reference: ppm refers to parts -per -million; bgs refers to below ground surface. A review of the analysis from the confirmation soil samples obtained from the former tank basin revealed EPH Aromatic fraction C-11-C22 at a concentration of 28.9 milligrams per kilograms (mg/kg) in sample OE-1 collected at the base of the excavation. The detected concentration of C-11-C22 is below the NCDEQ most stringent soil to groundwater MSCC of 31 mg/kg. No additional target analytes were detected in any of the confirmation samples exceeding the laboratory MRLs. The analytical results indicated that the sidewall excavation was completed within this guideline. The analytical results are summarized in Table 3 along with the criteria used by the NCDEQ, Division of Waste Management — UST Section. The laboratory analytical report and chain of custody is provided in Appendix C. J. Recommendations Based on the over excavation, removal of hydrocarbon affected soils and results of confirmation sampling results, PM recommends this incident be closed and the site granted no further action (NFA) in accordance with the NCDEQ UST Section guidelines.
