DATE AND TIME OF FIRE: 8/8/2019 @ 8:05AM
HOW WAS THE FIRE REPORTED AND BY WHOM:
Employees noticed when they arrived to work. Bud Tillett Noticed what looked to be smoke and stream coming from the top of the
mulch pile.
LIST ACTIONS TAKEN:
Bud call me ( Doug Huff) I then notified Fire Marshall Steve Kovac. When we arrived at the site found the mulch pile smoldering.
WHAT WAS THE CAUSE OF THE FIRE:
l'ppe"s to ha'e beeo a lightiog strike.

Mulch area Mulch smoldering and smoking just mulch pile involved.
WHAT COULD HAVE BEEN DONE TO PREVENT THIS FIRE:

DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS:
Going to remove piles of mulch.
