None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1. The purpose of this site visit was to visually inspect the LCID landfill unit.
2. Mr. Phillips stated that the Permit to Operate, approved Operations Plan and cover log had been returned to the
site office and were available for review if requested.
3. The entrance sign was not visible when approaching the facility from the northwest. Ensure that the
vegetation along the road frontage is trimmed back to make the sign visible from the roadway.
4. Maintenance had been performed on the entire landfill area since the previous inspection on June 11, 2019. Side
slopes had been reduced to an estimated 4:1 ratio. Mr. Phillips stated that Jeff Johnson, the landfill operator, had
received training on the operational requirements set forth in state law as well as the operating procedures that
make up the facility's approved Operations Plan.
5. The working face was small and manageable, and waste appeared to be well compacted.
6. No unacceptable material was observed in the working face or in the unprocessed waste staged for disposal.
7. Clean soil had been applied to the entire landfill operations area and tracked in thoroughly on the slopes.
Operational cover was good, and no exposed waste was observed.
8. Considerable progress had been made in removing waste stockpiles from the top of the landfill. Trees and logs
remained only on the northern side of the landfill top along the access road to the tipping area. Mr. Phillips
stated that he was working on removing the line of pine trees along the northern property line so that he could
stage wood waste operations there, as had been indicated on the new site plan drawing that had been approved
as a part of the permit renewal process. Ensure that the remaining wood waste stockpile is removed from
the landfill top and placed in the designated wood waste operations area northeast of the landfill unit.
9. Most of the excavation area had been regraded to prevent additional erosion on the slopes. Mr. Phillips stated he
would have the remaining grading work completed by week's end.
