OBSERVED VIOLATIONS:
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) An appropriate Septage Land Application Sign was present at the entrance to the site.
2) Roads were passable.
3) Some field markers were run over or missing. Any field markers knocked down or out of place need to
be restored.
4) No septage appeared to be discharged outside of permitted field boundaries.
5) No heavy applications or spot dumps were observed.
6) Field 3 has poor crop cover with a prevalence of weedy vegetation. No winter crop was discernible on
field 4. Some soil surfaces in permitted areas are crusted by past applications of lime mud contributing
to poor or no crop stands. Heavy vehicular traffic on the NE and eastern edge of field 1 and eastern
portions of field 3 has destroyed crop stands. Traffic on fields needs to be limited to only what is
necessary for land application of septage and maintenance of the crop. Lime mud and traffic affected
areas need to be restored and crop reestablished.
7) pH not checked. No application at time of inspection.
8) No objectionable odors noticed.
9) Setbacks not checked.
10) Records not checked.
