PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS:
OBSERVED VIOLATIONS:
An area on the western boundary/buffer area of field 2 extending outside of the permitted area was observed to be inundated by grease septage, where all vegetation was dead thus constituting a violation of the following rules: 15A NCAC 13B .0842 MANAGEMENT OF SEPTAGE LAND APPLICATION SITES (a) (11) all septage discharges shall be made at a location on the site consistent with the nutrient management plan; (a) (12) all septage discharges, including aerial drift from discharges, shall be made within the permitted boundaries of the land application site; (a) (14) grease septage shall be diluted at least 1:1 from its concentration when pumped with domestic septage or water if land applied over perennial vegetation. This dilution shall be increased if crop damage occurs. This dilution requirement shall not apply to the liquid portion of grease septage that has been treated to remove solids, fats, oils,
and grease as long as crop damage does not occur;
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) An appropriate septage land application sign was present at the entrance to the site.
2) Roads were passable in most areas; some areas of the access road were flooded and muddy.
3) Some field markers were run over or missing. Any field markers knocked down or out of place need
to be restored.
4) Septage was discharged outside of the field 2 western boundary.
5) An excessive amount of grease septage was applied on field 2 at the western boundary.
6) Areas of all fields have little to no crop cover. Soil surfaces in areas of all fields are crusted by past
applications of lime mud contributing to poor crop stands. Areas of all fields have areas where
weeds dominate existing vegetation. Traffic across the southern portion of field 4 and buffer has
destroyed crop. Weed control is needed; lime mud and traffic affected areas need Bermuda crop
restored. Vehicular traffic on fields should be minimized to what is necessary for land application of
septage and maintenance of the crop.

8) No objectionable odors noticed.
