PURPOSE OF SITE VISIT: Routine Inspection
STATUS OF PAST NOTED VIOLATIONS:
OBSERVED VIOLATIONS: A “Spot dump” was observed on Field 1 approximately 250 ft west of storage tanks on the boundary of the permitted area where an area was observed to be inundated with waste activated sludge covering an area approximately 90ft². The noted observation constitutes a violation of the following rule: 15A NCAC 13B .0842 MANAGEMENT OF SEPTAGE LAND APPLICATION SITES (a) (7) septage shall be applied to the surface of the land from a moving vehicle to have no standing liquid or soil disturbance resulting from the waste flow after the discharge is complete; (a) (11) all septage discharges shall be made at a location on the site consistent with the nutrient management plan;
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) An appropriate septage land application sign was present at the entrance to the site.
2) Roads where passable in most areas, some areas of the access road were flooded and muddy.
3) Some field markers were run over or missing. Any field markers knocked down or out of place need to
be restored.
4) Waste activated sludge was discharged on the boundary of permitted area west of storage tanks in a
manner violating permit conditions. (see observed violations pg.1)
5) Areas of field 1 and 2 have little to no crop cover. Soil surfaces in areas of field 1 and 2 were crusted by
past applications of lime mud contributing to poor crop stands. Fields 1 and 2 have areas where weeds
dominate existing vegetation. Weed control is needed and lime mud affected areas need Bermuda
grass crop stands reestablished
6) pH not checked. No application at time of inspection.
7) No objectionable odors noticed.
8) Setbacks not checked.
9) Records not checked.
