The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Met with Karry to gain access to the facility. The boiler is out of operation and any grease trap waste is
currently going (by flex hose) directly to the 10,000 storage tank located inside the building – bypassing the
screening box & outside tanks. No other significant changes in the facility or process were noted. Noted old
grease debris in the screening box which needs to be disposed of properly (the onsite dumpster is
overflowing with various trash). The 10,000 gallon storage tank located inside the building has overflowed at
some point (significant stain down the sides) and should be cleaned of all septage residuals. Discharge events
at this facility should be monitored by 301 personnel to ensure waste is handled appropriately.
2. The tanks appear structurally sound. The approved setbacks are being met. The required “No Trespassing” sign
is posted. The required 5 foot cleared radius (free of excess vegetation or debris) is present around the tanks.
3. Enclosed are copies of various items that are needed for the permit renewal for this dewatering facility. Please
submit the needed items so that I may proceed with the permit renewal.
