The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Noted an area of septage debris in front of the tanks which has been cut when the surrounding vegetation was
cut. This may be the remnants of a septage debris pile that was noted on the last visit of 2/27/2019. Clean up all
septage debris from the ground surface. No septage debris (waste, paper or plastic trash) from hoses, the
storage tank or truck tank may be placed on the ground surface. Discussed the situation with Frankie
Jones by phone and he indicated that he would address the situation.
2. The two tanks appear structurally sound.
3. The required signs are posted and the access road is adequate.
4. No recent spills or leaks were noted. It appears that septage is being transferred to/from the facility in a safe and
sanitary manner – other than the cut septage debris on the ground. No vectors or odors were noted.
5. The required 5 foot cleared radius (free of excess vegetation or debris) is present around the tanks at this time.
6. The approved setbacks are being met.
7. This Septage Detention or Treatment Facility Permit expires on November 19, 2019. The renewal application
shall be submitted by August 19th as per Rule .0837(m) as stated in the notification letter that was sent on July
1, 2019.
