Risk Classification: Low
The UST Section, Division of Waste Management (DWM), Mooresville Regional Office (MRO)
received the May 30, 2019 Phase I Limited Site Assessment on June 3, 2019. The paper correspondence
copy will be maintained in the MRO. The electronic copy has been loaded to the DWM's electronic
document storage system (Laserfiche).
A review of information on file with the Underground Storage Tank (UST) Section, Division of
Waste Management, Mooresville Regional Office indicates that soil and groundwater contamination meet
the cleanup requirements for a low -risk release under Title 15A NCAC 2L .0202 and .0400, but
contamination exceeds the limits allowed for the unrestricted use of a site where closed under risk -based
cleanup standards. Be advised that, as contamination may exceed the groundwater quality standards
established in Title 15A NCAC 2L .0202, groundwater within the area where contamination is present or
is expected to migrate is not suitable for use as a water supply.

Please note that before you sell, transfer, or request a "No Further Action" determination for a
property that has not been remediated to below "unrestricted use" standards, you must file a Notice of
Residual Petroleum ("Notice") with the Register of Deeds in the county where the property is located
(NCGS 14313-279.9 and 14313-279.11).

Please see the attached Notice of Regulatory Requirements Limited Site Assessment letter for GADO Building
(UST Incident #40833) located at 5318 Morris Field Drive, Charlotte, Mecklenburg County. I understand that Ed
Leach has been discussing filing a notice of residual petroleum on the parcel that the GADO Building site is
located upon to close multiple UST incidents. This site only requires a groundwater restriction. This parcel may
require a notice of residual petroleum with soil and groundwater restrictions to received closure at other sites.
