Active, Life of Site Permit issued March 27, 2019 and expires August 28, 2016. This permit is in the names of: Currituck County (owner) and Republic Services of North Carolina, LLC (operator).
PURPOSE OF SITE VISIT: Follow up Compliance Inspection.
STATUS OF PAST NOTED VIOLATIONS: Observed violations from the June 18, 2019 inspections have been resolved. OBSERVED VIOLATIONS: None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 – No windblown trash was noted during this inspection. 02 – Tipping floor and loading bay were clean.
03 – The only stored solid waste at the tipping floor were tires and LP tanks pulled from the waste stream to be taken to the designated areas for this solid waste. 04 – Matt Einsmann, Republic Services Environmental Manager has sent a corrective action letter on July 05, 2019 to outline actions taken and to be taken to keep this facility in compliance. Attached to this report. 05 – Facility has 3 certified operators: James Bolen, expires 10/09/2020, David Lowe, expires 05/25/2022 and Charlie Rome, expires 03/12/2022. 06 – One load was screened during this inspection and no non-conforming noted. 07 – Digital pictures taken during this inspection. 08 – Follow up inspections will continue to monitor this facility.

Response to June 2019 Compliance Inspection

trailer. On a daily basis, written documentation of this activity will be recorded, and a copy of the
documentation will be maintained at the facility.
In addition to the corrective actions cited above, a representative of Republic Services will periodically visit the
transfer station to ensure compliance with all rules and regulations. A copy of the form that will be used to
document compliance with the rules and regulations is attached to this letter.
