PURPOSE OF SITE VISIT:

STATUS OF PAST NOTED VIOLATIONS:
.0838(j) - still observed an ongoing spillage issue at tanks #2 & #3, septage debris on the tops, sides and ground
surface around these two tanks.
OBSERVED VIOLATIONS:
.0838(j) Septage shall be transferred to and from a detention facility ins a manner that prevents leaks of spills of septage
onto the ground surface or exterior of the detention system, including septage in pipes used for transferring waste to and
from vehicles. Permit Condition 2 Leaks and spills are the responsibility of the Permittee and shall be immediately
contained, cleaned up, and treated to prevent any contamination of the surrounding area. Noted a significant amount of
septage debris on the tops, sides and ground surface around tanks #2 & #3. Completely clean up the top, sides and
ground surface around tanks #2 & #3 of all septage debris/residuals and dispose of the waste properly. (On 7/3
Manny Perez stated that the area had been cleaned up). While the storage tanks are in use action should be taken
to prevent any spills and if there is a spill/overflow it shall be cleaned up immediately.

ADDITIONAL COMMENTS
1. Construction is ongoing at this time in regards to the permit modification. Tanks 101 & 102 have been placed in
the containment area but the lines are not installed yet. The concrete containment area is finished. Processing
(of septage) is occurring directly at tanks #2 & #3 (the new screening box is temporarily in place at these tanks).
Storage tanks #5, #6 & #7 are being used for septage storage which will be sent to the Winston Salem facility.
All the old tanks will be cut up & taken to the landfill for disposal. Additional review of the operation manual is
needed and final site drawings. The discharge permit to the sewer line will not be issued (by PWC) until after
construction of the facility is complete.
2. The required sign is posted and the access road is adequate.
3. No significant odors or vectors noted.
4. The above ground tanks appear structurally sound.
5. The approved setbacks are being met.
6. The required 5 foot cleared radius is present.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
