The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle – Tag#145C338, VIN#$V4DWC9DH4KN199067, 4200 gallons. The tank appears
structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is
correct on both sides of this vehicle per the information on the submitted firm application. This vehicle is in
compliance at this time.
2. Inspected three other vehicles that were on site at this time.
Tag#073C291, VIN correct per the application, tank is structurally sound & securely attached. Outlet covers
were on but a slight drip was noted at the center outlet – instructed Mr. Mayberry & Mr. Perez to get a
bucket under the drip until the outlet could be inspected & repaired as needed. The lettering is correct on
both sides of this vehicle.
Tag#149C092 (used to be tag#078C329), VIN correct per the application, tank is structurally sound & securely
attached. Outlet covers on, no drips/leaks noted. The lettering is correct on both sides of this vehicle.
Tag#K640976 – this vehicle was not listed on the submitted firm application. Tank is securely attached &
structurally sound. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of
this vehicle.
3. Two vehicles listed on the 2019 firm application (for the Fayetteville location) were not inspected during this
visit. Tag#K041187 is temporarily located in Charlotte at this time. Tag#078C328 is now permanently located
in Winston Salem. Ensure that the application submitted this winter for the firm permit renewal has all of
the correct vehicles listed for this location.
4. Truck logs were not reviewed at this time.
