STATUS OF PERMIT:
A Permit to Operate Existing Landfill Unit & Horizontal Expansion Unit was issued to Louisiana-Pacific Corporation
on February 4, 2016. This permit shall expire on December 28, 2019.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The Louisiana-Pacific Corporation manufactures hardboard (construction) siding and operates a wood ash landfill
located adjacent to the manufacturing facility.
2. The landfill is permitted to accept wood ash generated from the wood-fired boilers and zinc borate sludge from the
facility’s wastewater treatment plant.
3. The facility is required to maintain a record of the amount of solid waste received at the landfill unit, compiled on a
monthly basis. Disposal records for July 2018 through May 2019 were verified. The facility received a total of
8,732-tons of waste during this time.
4. Groundwater monitoring was being performed semiannually as required. Facility staff take samples which are sent
to Meritech, Inc. Environmental Laboratory for testing. Second semiannual sampling for 2018 (performed August
21, 2018) and first semiannual sampling for 2019 (performed February 26, 2019) were verified. Please ensure that
sampling results are submitted to the Solid Waste Section with 120-days of the respective sampling event as
required.
Operations Inspection of the Landfill Unit:
5. Waste was being placed in Phase 3B at the time of inspection.
6. Inactive portions were stabile with native vegetation. The top of the landfill had been recently mowed, but the
slopes of the landfill were still in need of mowing preventing a thorough inspection of the landfill slopes.
7. Multiple fire boxes equipped with hydrants and hoses were provided on top of the landfill to aid in fire control.
8. The facility is approved to mix zinc borate sludge with ash for disposal in the landfill. The sludge to ash ratio is
required to be an average of 2:1.
9. A small area of bare vegetation was observed on the west side of the top of the landfill. The area was
approximately 20-feet wide x 30-feet long. Please ensure that this area is seeded and stabilized with a groundcover
sufficient to restrain erosion.
10. A small slope slide was observed at the top of the south facing slope approximately 60-feet from the riser basin.
Please ensure that the slope is repaired and stabilized with a groundcover sufficient to restrain erosion. The slide
was small at the time of inspection but could worsen if not immediately addressed.
11. A depression was observed on the landfill at the crest of the south facing slope near fire box #37. The depression
measured approximately 3-feet long x 2-feet wide x 1 ½- feet deep. Please ensure that this depression is repaired
and stabilized with a groundcover sufficient to restrain erosion.
12. A diversion with exposed material was observed on the fill bench just above the south facing slope which leads to
the riser basin on site. Slight erosion was observed within the diversion. It is recommended that this diversion be
stabilized in order to carry flow in a non-erosive manner.
13. The working face was well controlled at the time of inspection.
14. Vegetation had taken root in the impoundment area of the riser basin as noted on the previous inspection report. It
is recommended that all vegetation be removed from the riser basin and the basin be restored to original storage
capacity.
15. Edge of waste markers were in place.
16. The facility is secured by a gate and access must be granted via scale house personnel.
17. All access roads were of all-weather construction and well maintained.
