Solid Waste Director James Bolen – Certified Transfer Station Operations Specialist – Bowen John College – NCDEQ-DWM-SWS-FOB STATUS OF PERMIT: Active, Life of Site Permit issued March 27, 2019 and expires August 28, 2016. This permit is in the names of: Currituck County (owner) and Republic Services of North Carolina, LLC (operator). PURPOSE OF SITE VISIT: Follow up Inspection at opening of facility. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: 15A NCAC 13B .0105 COLLECTION AND TRANSPORTATION OF SOLID WASTE 15A NCAC 13B .0402 OPERATIONAL REQUIREMENTS FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS: 01 - This facility follow-up inspection was conducted at 7:00 am when gate was first opened to ensure compliance had been met from operations on June 17, 2019. 02 - Upon arrival, Mr. James Bolen, Transfer Station Operation Specialist and facility Supervisor was present. 03 - Leighton Earwood (Currituck Co. Solid Waste Director) was notified of my presence at the facility. 04 - Upon arrival at the tipping floor it was noted that the floor had not been cleaned at the end of the day and there were items pulled out of the waste stream along both sides of the inner walls, scavenging was in question. This was confirmed by Mr. Bolen. 05 - It was noted that the windblown trash had not been picked up on June 17, 2019 and possibly longer. This was confirmed by Mr. Bolen. 06 - It was noted that the loading bay of the tipping floor also had not been cleaned on June 17, 2019 and possibly longer. This was confirmed by Mr. Bolen. 07 - It was noted that trailer # 173 (new trailer) at the loaded trailer area was leaking leachate on the ground with available troughs not being used. 08 - At this point Leighton Earwood (Currituck Co. Solid Waste Director) arivied to this facility. 09 - After John College and Leighton Earwood talked about the issue found, Michael Kavanaugh (Republic Services Divison Manager) was called and a voicemail was left by Mr. Earwood. Matt Einsmann (Republic Services Environmental Manager) was also notified by John College. Voicemail left. 10 - Some the issues noted above were noted on the February 12, 2019 inspection report and these issued were resolved on the March 12, 2019 inspection report but have now reverted back to non-compliance. 11 - Mr. Bolen notified Stacy Bowen (Sub Contract Operator) and he sent David Lowe (Bowen Regional Supervisor) to facility to immediately start measures to get this facility into compliance. 12 - Michael Kavanaugh (Republic Services Divison Manager) came to the facility to meet with Bowen Sub-contractor staff to ensure compliance was met. 13 - Matt Einsmann (Republic Services Environmental Manager) and Stacy Bowen (Bowen Co. LLC) called John College to discuss the above issues to ensure compliance will be met at this facility. Staff termination was discussed by Mr. Bowen. 14 - Screening records were available and reviewed. Records were complete and up to date. 15 – Incoming load tickets have required information. 16 – Leachate tanks were pumped out on April 1, 2019. Please keep yearly records on-site. 17 – Yard waste has been 90% ground for removal to Soundside Recycling to be composted. 18 – White goods and scrap metal area looked good. 19 – Scrap tires & LP tanks have been pulled out of waste stream at the tipping floor and are recorded on screening records. 20 – Corrective is needed at this facility due to non-compliance items listed on this report. 21 – Digital pictures were taken during this facility inspection. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
