FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected one vehicle listed under this firm permit. Tag#YA016246, VIN#1HTSDAANXYH263383, 2500 gallons. The tank appears structurally sound & securely attached. The lettering was correct on both sides of this vehicle per the information listed on the firm application. The lettering was slightly damaged but still legible at this time. When the vehicle is painted in the future an inspection will be needed in regards to the new lettering. 2. Outlet covers were on but a slight drip was noted at the outlet. Stated that the cause of the leak needed to be determined & adjustments/repairs made as needed to stop the leak. No liquid from the tank contents shall drip/leak onto the truck or ground surface. 3. The other permitted vehicle Tag#EM4105 is currently in the shop and not available for inspection at this time. 4. Received a copy of the truck logs for review.
