Expires: 1/13/2023 PURPOSE OF SITE VISIT: Follow-up Meeting/Inspection STATUS OF PAST NOTED VIOLATIONS: 15A North Carolina Administrative Code (NCAC) 13B .0838(j) states: Septage shall be transferred to and from a detention system in a manner that prevents leaks or spills of septage onto the ground surface or exterior surface of the detention system, including septage in pipes used for transferring waste to and from vehicles.
Triple T Pumping was in violation of 15A NCAC 13B .0838(j) because septage from the dewatering facility operating under Septage Management Permit Number SDTF-95-01 flowed onto the ground on April 11, 2019.
Resolution is pending implementation of additional leachate control measures.
OBSERVED VIOLATIONS:

ADDITIONAL COMMENTS 1. The Septage Detention and Treatment facility (SDTF) consisted of one 100-gallon, two 4,000-gallon, three 1,000-gallon, one 6,000-gallon, one 10,000-gallon and one 12,000-gallon tanks. The nine tanks have a total capacity of 39,100 gallons for the storage of untreated and treated waste. In addition, one 6,000-gallon dewatering drum and one 30-yard dewatering box is on site.
2. The meeting was scheduled to discuss an appropriate course of action to prevent leaks or spills of septage onto the ground or off site. Participants were Troy Harrison, Mike Garlock, Triple T and Andrew Blethen, Appalachian Health District.
3. No leaks or spills were observed. The sump pump that is located where the dewatered sludge and wood chips are mixed was in operation during the inspection. Mr. Galock stated that the sump operates on a timer but can be
turned on in the event of heavy rains. 4. Two new tanks have been purchased to replace existing ones. One was observed on site but not installed yet. An inspection after installation is required; contact Troy Harrison to schedule an inspection. 5. Additional actions to be taken are: A. A daily safety checklist is to be maintained by Triple T that includes inspection of the tanks for leaks and the sump pump. The checklist must also include any corrections made to prevent spills or leaks and any additional notes that are necessary for clarification. This is to be implemented immediately.
B. A berm is to be constructed at the below the facility to prevent overflow into the culvert. This must be completed by June 14, 2019. 6. Storm water from areas not covered under the septage permit, such as a mulch pile that is not mixed with septage, are not regulated by the division of waste management.

Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
