OBSERVED VIOLATIONS:
1. .0835(b) All permitted septage management firms shall display lettering on each side of every pumper vehicle
operated by the firm. The lettering shall include the firm name, town name, phone number, and septage
management firm permit number as shown on the firm application. All lettering required by this Rule on the
pumper vehicle shall be no less than three inches in height and legible, distinguishable from the background, and
not obstructed from view. Inspected a new vehicle to be added to this permit, tag#JM2589. The town name on
both sides of this vehicle has letters that are less than the required three inches in height. Correct the town name
on both sides of this vehicle at three inches or greater in height. Submit a photo within 10 days verifying
that the lettering has been corrected on both sides of the vehicle (display a ruler in the photo for reference).

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected a new vehicle to be added to the firm permit. Tag#JM2589, VIN#5PVMJ8JL4K4S52243, 1100
gallons.
The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted.
2. Lettering violation listed on the front page of this report. A pump vehicle can be used for septage pump jobs
prior to an inspection by this office only if the vehicle is in compliance with Rule .0835 which is the
responsibility of the permit holder to determine (as in verifying/measuring the required lettering upon receipt
from the sign company). The lettering on this vehicle was not in compliance.
3. No other pump vehicle was inspected at this time and truck logs were not reviewed.
