FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected two pump trucks, Tag#JP 8035, VIN#JNAPA33H32AG65153, 1100 gallons and Tag#1 0721A (Alabama tag), VIN#4V1JBBME2PR823522, 2300 gallons. When the Alabama tag is replaced with a NC tag inform the Raleigh office or myself of the new tag number. 2. The tanks on the two vehicles appeared structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides of both vehicles (per the information on the submitted application). 3. Gave an example truck log & discussed the information to be maintained. Gave a 2019 class schedule – attend one of the locations to obtain the required 4 firm hours for permit renewal this winter. 4. After the inspection I left messages for Jeff Bullard & Chester Cobb at the Raleigh Central office to inform them that the vehicles passed inspection. No septage pump jobs shall occur until the firm permit has been issued by the Raleigh Central office. Please contact me if you have any questions or concerns regarding this inspection report. _ Phone: (910) 433-3352 Connie S.
