ADDITIONAL COMMENTS
1- Inspected second vehicle to be added to firm permit. Tag#HAV1113, VIN#1FDWF36S6XEC75951, 450
gallons. Tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were
noted. The lettering is correct on both sides of this vehicle. This vehicle is in compliance at this time.
2- An additional permit fee of $250 is required for this second vehicle and shall be received by the Raleigh
Central office before this vehicle is utilized for septage pump jobs. Currently this vehicle is under repair and
not in use.
3- Truck logs were not reviewed at this time and no other pump truck was inspected. Gave Mr. Locklear a 2019
class schedule and he stated that he planned to attend the Asheville location in September.
