Permit to Operate 3217-MWP-2019, Wall Recycling Holman Street Facility, was issued February 8, 2019 and will
expire on February 8, 2024.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
The facility is permitted to receive construction and demolition debris waste. The facility is also approved to accept
land clearing debris, wooden pallets, yard trash, clean soil, and cardboard. The Mixed Waste Processing facility is
designed to handle a maximum average rate of 1,500 tons per day.
1. During the inspection, operations of the Mixed Waste Processing facility had not yet begun.
2. The facility has a large concrete pad that is the tipping floor and transfer trailer loading area. Concrete walls
surround part of the concrete pad and more walls are going to be installed soon.
3. The facility is secured by means of gates and fences to prevent unauthorized entry. The fence will also help
control windblown waste, which will be collected daily.
4. Control measures will be utilized to minimize and eliminate visible dust emissions.
5. Interior roadways are of all-weather construction and maintained in good condition.
6. During a rain event, the facility plans to minimize site activity and use tarps to cover waste on the tipping floor.
7. C&D waste will be processed and then loaded into trailers for transportation to the landfill.
