Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1 – Area behind office building 2 – Trench area
3 – Area where trench bent towards side of property 4 – Leveling of entire area after clean-up
1 – Called PVI Enterprises and spoke with James Faulk. Informed him that follow-up visit would be performed that day. Arrived approximately 9:45 am. 2 – Picture one (1) is of the area behind the office building showing where trucks had backed up to and previously washed out. 3 – In second (2) picture, the trench leading from the truck wash area had been limed and filled in.
4 – The third (3) picture shows the rest of the trench leading towards trees also filled in. 5 – Lastly, picture four (4) shows the entire area has been cleared of debris, filled in and leveled. 6 – Spoke with Chris Faulk at the site about not using the area for truck cleaning in the future. 7 – Spoke with James Faulk about the Notice of Violation and that possibly more paperwork to follow.
