Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1 – Area behind office building 2 – Trench area 3 – Area where trench bent towards side of property 4 – Leveling of entire area after clean-up 1 – Called PVI Enterprises and spoke with James Faulk. Informed him that follow-up visit would be performed that day. Arrived approximately 9:45 am. 2 – Picture one (1) is of the area behind the office building showing where trucks had backed up to and previously washed out. 3 – In second (2) picture, the trench leading from the truck wash area had been limed and filled in. 4 – The third (3) picture shows the rest of the trench leading towards trees also filled in. 5 – Lastly, picture four (4) shows the entire area has been cleared of debris, filled in and leveled. 6 – Spoke with Chris Faulk at the site about not using the area for truck cleaning in the future. 7 – Spoke with James Faulk about the Notice of Violation and that possibly more paperwork to follow.
