The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. As a reminder, the facility's Permit to Operate expires on December 11, 2019 and the permit renewal
request must be submitted to the Solid Waste Section by June 10, 2019. Please contact Ming-Tai Chao,
Permit Engineer, for assistance at 919-707-8051 or via email at Ming.Chao@ncdenr.gov.
2. The purpose of this inspection was to follow up on site improvements that were in progress during the previous
inspection on March 5, 2019. At that time, ponding water had been observed in and around the windrows and
facility erosion control devices had been overwhelmed.
3. The facility appeared to be clean and well -maintained. Access roads were found to be of all-weather
construction and free from equipment ruts and ponding water.
4. Following the previous inspection, facility staff consulted City of Winston-Salem Erosion Control Inspector
Neil Uldrick to discuss the best course of action. Mr. Teague stated that he had been advised not to breach the
sediment pond, located in the southern corner of the property. He explained that Mr. Uldrick preferred the pond
remain intact and that, when the pond emptied and dried, the rip -rap lining be removed and replaced, and the
pond cleaned and cleared of sediment and debris. Mr. Teague remarked that, with the recent spate of dry
weather, the level in the pond had already been greatly reduced.
5. The pond appeared to be in a condition similar to that of the previous inspection on March 5, 2019. However,
the drainage area leading up to the pond had also been regraded and now appeared to be in a much more stable
condition.
6. The check dams, located along the western side of the property, appeared to be functional and were dry and
clear of debris.
7. Most of the site had been regraded, and the mulch piles rotated. No water was found to be ponding in and
around the windrows.
8. Windrows were neatly piled, with adequate space between them to accommodate facility equipment and
emergency access, if needed.
9. No unacceptable material was found in the processed and unprocessed wood waste stockpiles.
The facility appears clean and well -maintained; windrows are neatly piled with adequate fire lanes.
