Box 390 Lumberton, NC 28359 GPS COORDINATES: N: 34.70635 E: -79.03776 FACILITY CONTACT NAME AND PHONE NUMBER: Name: Michael Cody Hunt Telephone: (910) 738-5311 Email address: mchunt22@gmail.com Fax: (910) 738-5563 FACILITY CONTACT ADDRESS: 3330 Saddletree Road Lumberton, NC 28360 PARTICIPANTS: Connie Wylie, Matthew Markus STATUS OF PERMIT: Active – permit expires August 24, 2023 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: In regards to the effluent violation noted on the last audit of 3/08/3019, the dewatering box was again discharging effluent during this visit. The effluent was being properly contained and was correctly flowing thru the lines/tanks at the facility. OBSERVED VIOLATIONS: N/A SEE BACK OF PAGE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. An overflow spill was noted on primary storage tank #4 (stains on the sides of the tank). Did not definitely see septage on the ground surface but a dark area was observed. The top & sides of this tank should be cleaned of all waste and any residuals removed from the ground surface. Mr. Markus stated that the tank would be cleaned. 2. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No active leaks or spills noted. The overall cleanliness of the facility has improved. 3. The above ground tanks appear structurally sound. 4. The required signs are posted and the access road is adequate. 5. The approved setbacks are being met. No apparent physical change in the facility. 6. The required 5 foot cleared radius (free of excess vegetation or debris) was present around the tanks & pit. 7. No significant odors noted at this time. Flies were noted at the solidification pit. 8. The solidification pit was full of septage & mulch. Also noted some plastic bottles & general trash in the pit. Although the solidified waste goes to the landfill for final disposal, this pit is permitted for septage (as in dewatered solids) and mulch, not household trash and as such no general trash should be dumped in this pit. 9. Regular monitoring/cleanup of the area around the pit should occur to ensure no septage or contaminated mulch is on the top or sides of the concrete walls or on the adjacent ground surface. The posts marking the designated area for the mulch stockpiles were present.
