NC 28115 January 21, 2019 Professional Engineer Statement Underground storage tank closure activities were conducted at Windstream Communications — Sardis Road North site located at 1813 Sardis Road North in Charlotte, NC by C.E.R.T. PO Box 681016, Charlotte, NC 28216. All specifications and operating procedures utilized were in accordance with good engineering practices. Soil sampling was performed by me at appropriate locations utilizing guidance outlined in Underground Storage Tank Section Guidelines for Site Checks, Tank Closure. and Initial Response and Abatement, May 2017, and as shown in the UST Closure Report (UST-12). The laboratory analysis, performed by a state certified laboratory, indicates NO residual contamination above "State Action Levels". Based on this and information known of this site, we believe that the site should be subject to "no further action". CAF'J0, 1. p, Jzt 1 Zol9 R. Osborne, PE Providing Environmental Engineering, Technology and Service Limitations These environmental services have been performed for the exclusive use of our client for the specific application to this project. The specifications and operating procedures utilized in the performance of these services is in accordance with generally accepted engineering principals. No other warranty, expressed or implied, is made. Although we cannot be responsible for the accuracy of the data supplied by others we have no reason to suspect that any of the information provided is inaccurate unless it has been noted otherwise. Our observations are based upon conditions visible at the site at the time of our visits. Chemical analyses were performed on selected soil samples to determine the presence and/or concentrations of potential contaminants. Selections of specific EPA test methods for chemical analyses were based upon (1) information supplied by our client (owner) concerning raw materials, products, and/or wastes which may have been handled or generated within the scope of this project, and (2) guidelines provided by the North Carolina Department of Environmental Quality. Conclusions presented in this report are based upon the services described, not on specific research, tasks, or procedures beyond the scope of described services; nor, beyond the time and/or budgetary constraints imposed by our client. Information about a release; There are no documented releases at this location. B. Site history and characterization; 1. UST owner and operator information; Windstream Corporation 4001 North Rodney Parham Road Little Rock, AR 72212 (501) 748-4812 2. Provide UST information; See the attached table. 3. Provide non -UST information; A new generator with a self-contained fuel tank is located on the site. 4. Describe relation of location of current UST system to historical UST releases, non -UST releases, and off -site releases; There are no documented releases at this location. 5. Site Characteristics; This site contains a service facility for Windstream Communications. Soil encountered consisted of red/brown silty clay. No indication of the depth to groundwater or bedrock was noted in the excavation as these were not encountered. The site is served by the City of Charlotte water and sewer system. C. Closure procedures; 1. Describe preparations for closure including steps taken to notify authorities, Permits obtained, and steps taken to clean and purge the tank. Authorities were notified prior to the removal and all necessary permits were obtained. A UST-3 Notice of Intent -UST Permanent Closure or Change in Service form was sent to the North Carolina Department of Environmental Quality on 6/8/18 (see Appendix A). The City of Charlotte fire marshal was notified and a permit for the removal was obtained (see Appendix E). The fire marshal was not present for the tank removal project. Approximately 600 gallons of diesel fuel from the underground tank was removed. A copy of the manifest is present (see Appendix C). 2. Describe the closure procedures; The Health and Safety Plan (see Appendix D) was reviewed and signed by all personnel present. All liquid had been removed from the tank prior to the removal. A small concrete berm and soil were removed down to the top of the tank. Soil was removed from both sides and ends of the tank. The fiberglass tank showed no deterioration and no holes were present in the tank or lines. The tank was placed on a trailer and removed for disposal. The 1/2 inch copper lines were removed up to the edge of the new generator area. Windstream Communications -Charlotte, N. C. UST Closure Report-1/19 Although these lines were encased in PVC, a soil sample was taken from below them. No petroleum odor was noted at any location around the tank cavity or line area. Laboratory results also show no contamination present. (see Appendix B). See the attached map for sample locations (Figures). 3. Note the amount of residual material pumped from the tank and describe the storage, sampling, and disposal of the residual material and the disposal of the tank(s), pumps, and piping; No residual material remained in the tank at the time of the removal. Approximately 600 gallons of diesel fuel had been removed prior to the digging. A copy of the liquid manifest is present. The tank was disposed of by the Carolina Environmental Response Team. A copy of the Certificate of Disposal is attached (see Appendix Q. 4. Document any soil excavation activities; The tank was buried to a depth of 4 feet. As the soil was removed, checks were made to determine the presence of soil contamination. The tank area was 9 feet by 16 feet. The depth to the bottom of the tank was 8 feet. Approximately 15 tons of soil was removed to access the tank. An additional 15 tons of backfill was added to complete the removal. Backfill consisted of clean soil from a local quarry. No groundwater, free product or bedrock was encountered during the excavation. D. Site investigation; 1. Field screening activities; The soil was red/brown silty clay. No petroleum odor of any type was noted in the soil beneath and beside the UST or product and return lines. The soil samples were taken in native soil below each end of the tank and from below the copper lines with a portion of the soil being split into plastic bags for field checks via the PID. A Mini Rae 2000 Photo -ionization Detector was used to field screen the soil samples. The PID was calibrated to display concentration in units equal to parts per million. First, a supply of zero air, which contains no ionizable gases or vapor, is used to set the zero point. Then, span gas containing a known concentration of a photo -ionizable gas or vapor is used to set the sensitivity. Isobutylene at 100 ppm was used as the span. The field screening was done by holding the PID next to the soil that had been removed and observing the meter to determine if contaminated soil was present. In addition, soil samples were placed in plastic bags and allowed to warm and then were read. None of the checks indicated the presence of soil contamination. 2. Document soil sampling information; The soil samples were collected from the open excavation by use of the back hoe digging an area approximately two feet below each end of the tank along the centerline of the tank. Fresh nitrile gloves then removed soil that had not come into contact with the track hoe bucket. This soil was placed in new laboratory provided containers/preparations. Windstream Communications -Charlotte, N. C. UST Closure Report-1/19 2 The tank soil samples were collected at a depth of 10 feet below ground level. The line samples were collected from approximately 2.5 feet deep, below the PVC encased copper lines. See the attached map for the sample locations. 3. Document groundwater and surface water sampling information; No groundwater or surface water was encountered during the excavation. 4. Document quality control measure information; Upon removal, each sample was prepped for GRO via 5035 prep. and DRO. Laboratory provided containers were used for sampling. The samples were placed in a chilled cooler and taken to the lab. Fresh disposable gloves were used to handle each sample. See the attached chain of custody for the date and time that the samples were submitted to the lab (see Appendix B). As samples from various points around the area were removed, they were field checked for soil contamination. These samples were placed in plastic bags, allowed to warm, and checked. 5. Describe soil and groundwater investigation results; See the analytical results attached (see Appendix B). All of the sample results are shown as non -detect. No groundwater or bedrock were encountered during the excavation. The samples were checked for GRO via 5030 prep and DRO. PID readings showed no soil contamination present. E. Conclusions and recommendations; Upon receipt of the analysis of soil sampling, it was determined that no soil contamination exists at this site. No groundwater, bedrock, or free product was encountered during the excavation. On this basis the property should be subject to "no further action". Windstream Communications -Charlotte, N. C. The UST-2 form should also be submitted to the Central Office in Raleigh so that the status of the tanks may be changed to permanently closed and your tank fee account can be closed out. UST closure and change -in-service site assessments must be completed in accordance with the latest version of the Guidelines for Site Checks, Tank Closure and Initial Response. Note: To close tanks in place you must obtain prior approval from the DWM Regional office located in the region where the facility is located. You must make sure that USTs removed from your property are disposed of properly. When choosing a closure contractor, ask where the tank(s) will be taken for disposal. Usually, USTs are cleaned and cut up for scrap metal. This is dangerous work and must be performed by a qualified company. Tanks disposed of illegally in fields or other dumpsites can leak petroleum products and sludge into the environment.
