The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Reviewed submittal for permit renewal – tweaked application & nutrient management plan. Mr. Oaks will be
submitting a copy of the field logs from August 2018 until now.
2. Mr. Oaks wishes for Field 1 (which has been inactive for some time) to become active again with the new
permit. An adequate cropstand of fescue will have to be reestablished on this field and approval granted by this
office before land application may occur on Field 1. Setbacks/field markers will also have to be re-established
on this field. Currently this field is bare but Mr. Oaks intends to plant fescue sometime this year.
3. Field 2 (the active field) has been sprayed for weeds & needs harvested soon. Mr. Oaks stated that it will be
determined whether or not any reseeding of fescue is needed on this field. Noted scattered areas of what appears
to be damage from hogs on this field.
4. Field markers are still missing at the swale on Field 2. Mr. Oaks stated that no land application is currently
occurring on this field. Before any septage is discharged on Field 2 the required field markers will be posted at
the swale. Permitted boundaries for this field shall be clearly marked on the ground.
5. It appears that the approved setbacks are being met but were not measured at this time. No sign of land
application outside the permitted boundaries. No sign of heavy or recent application. No odors noted.
6. The required signs are posted and the access road is adequate.
