None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. A gate was located at the entrance to the facility to prevent unauthorized access.
2. The landfill was stabilized with native vegetation at the time of inspection.
3. Edge-of-waste markers consisting of PVC pipes placed over steel stakes were in place at the time of inspection.
The marker located at the northwest corner, adjacent to some concrete buttresses had fallen over and needs repair.
This condition was observed during the previous inspection.
x As noted on the previous inspection report, additional edge-of-waste markers are needed along the
southern perimeter of the landfill. One steel stake was observed in the wooded area south of the landfill,
but it could not be confirmed if this stake was marking the edge of waste.
4. During the previous inspection, significant ponding was observed on the north side of the landfill adjacent to the
scalehouse. The area measured approximately 12-feet X 25-feet. No ponding was observed on the landfill during
the inspection performed on April 11, 2019.
5. No areas of significant settling were observed.
6. During the previous inspection, tall vegetation and vines were observed around the methane monitoring well
located near the center of the landfill. It was requested that vegetation around the well be cut back and maintained.
x No tall vegetation was observed around the methane well during the inspection performed on April 11,
2019. Mr. Saltz explained that the grass on the landfill had been cut for hay in September of 2018.
However, vines could still be seen growing on the well. Please ensure that all fines are cut away from
the well.
7. An area of tall grass and briars was observed just south of the methane monitoring well. The area did not appear to
have been mowed in quite some time, and it appeared to be a storage area for facility equipment. During the
previous inspection, some areas of bare vegetation were also observed in this location. Inspection on April 11,
2019 found that previously observed bare areas had been stabilized with vegetation. However, as noted on the
previous report, it is recommended that all facility equipment observed in this area (rebar, PVC Pipe, 4”,
and corrugated plastic pipe) be removed so the area can be properly maintained.
8. Multiple trees appearing to be less than 6-inches in diameter were observed growing on the landfill within the
edge-of-waste markers. All trees must be removed.
9. Groundwater monitoring was being performed semiannually as required. Reports for 1st semiannual sampling
events for 2016 and 2017 along with 1st and 2nd semiannual sampling events for 2018 were verified on the Solid
Waste Section online database.
x Reports for 2nd semiannual sampling for 2016 and 2017 were not found. Please confirm submittal of these
reports. If not previously submitted, please submit the reports for review.
x Review of monitoring data indicates that methane is being monitored at the methane well located at the
center of the landfill and at groundwater monitoring wells MW1 – MW5. Please contact Mr. Perry
Sugg, Hydrogeologist, to discuss whether monitoring for methane at groundwater monitoring wells
is still an adequate method and submittal of a revised Methane Monitoring Plan. Mr. Sugg can be
