OBSERVED VIOLATIONS:
Rule .0835(b) All permitted septage management firms shall display lettering on both sides of every pumper vehicle
operated by the firm. The lettering shall include the firm name, town name, phone number, and septage management firm
permit number as shown on the firm application. All lettering required by this Rule on the pumper vehicle shall be no less
than three inches in height and legible, distinguishable from the background, and not obstructed from view.

Inspected new vehicle (tag#SC:P606265) to be added to the firm permit. Some of the numbers in the permit number
(1&7) are less than three inches in height on both sides of the vehicle. Correctly display all numbers in the permit
number at three inches in height or greater. Rick stated that he would send me a photo of the corrected lettering.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected another vehicle to be added to this firm permit. Tag# SC: P606265, VIN#1GC4KYC81FF164472,
400 gallon waste capacity. The tank appears structurally sound & securely attached. Outlet covers were on and
no drips/leaks were noted. The lettering was correct except for part of the permit number.
