OBSERVED VIOLATIONS:
Rule .0835(b) All permitted septage management firms shall display lettering on both sides of every pumper vehicle
operated by the firm. The lettering shall include the firm name, town name, phone number, and septage management firm
permit number as shown on the firm application. All lettering required by this Rule on the pumper vehicle shall be no less
than three inches in height and legible, distinguishable from the background, and not obstructed from view.
Inspected second vehicle (tag#YA149715) to be added to the firm permit. The town name (Creedmoor) on both sides of
the vehicle was less than three inches in height. Correctly display the town name at three inches in height or greater
on both sides of the truck. Mr. Currin stated that he would send me a photo of the corrected lettering.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected the new vehicle - tag#YA149715, VIN#1FVACXDC27H237537, 2,000 gallon capacity. The tank
appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The
lettering is correct except for the town name which needs to be corrected.
2. The older vehicle is currently out of service/in the shop and was not inspected at this time.
3. Reviewed the truck pumping logs – the example log is being used and it appears that the required information is
being maintained except that the full physical address is not listed every time (town name is missing on some
jobs). Begin recording the full address (including the town name) for each pump job.
4. The 2019 firm permit does not have the correct firm name (Currin Septic Service) per the application submitted
by Mr. Currin and as displayed on the vehicle. I informed the Raleigh office of this issue and it was corrected
with a new 2019 firm permit.
5. Discussed grease trap waste & disposal options. Enclosed is a disposal list.
