Active Life of Site Permit issued May 10, 2018. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 - Facility was issued a Life of Site Permit on May 10, 2018.
02 - Facility is an approved C&D recycling center & waste transfer station. 03 - Gypsum pad and Asphalt shingle areas approved in 2014. 04 - Facility is secured with fencing and a locking cable.
05 - Facility has 5 Certified Transfer Operations Specialist: Kimberly Newbern, expires 04/14/2012, Horatio Newbern IV, expires 04/14/2021, John Jamerson, expires 06/21/2021, Michelle Farrow, expires 06/01/2021 and Rashell Bustamante, expires 06/01/2021. 06 - Waste screening records were reviewed and were found to be up to date. 07 - One load on the tipping floor was screened while inspection was being conducted. Approved waste only. 08 - Incoming load tickets were reviewed and has required information. 09 - Waste is approved from Currituck and Dare Counties. 10 - Non painted Pallets from non-construction waste are being source separated and ground as mulch. 11 - Due to high winds the night before this inspection, slight windblown trash noted. To be picked up by end of day. 12 - Scrap metal pile OK. 13 - Operating Hours are: 07:30 am to 05:00 pm weekdays and 07:30 am to 12:00 pm on Saturdays. No operations on Sundays. 14 - Yard Waste area appears to be managed well.
15 - Contingency plan reviewed. 16 - Facility is permitted to receive the following waste types: C&D, LCID, Asphalt, Wood Pallets, Yard Trash, Post Consumer Tear Off Shingles and Tires.
17 - Overall facility appears to be managed well. 18 - Digital pictures taken during this inspection. Several attached to this report.
