The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

GENERAL CONDITIONS:
1. The facility is permitted for the treatment of medical waste with steam sterilization, as well as for the storage
and processing of treated and untreated medical waste.
2. Prohibited wastes include: hazardous wastes, radioactive wastes, and liquid wastes.
3. The facility receives medical waste from across the southeast US.
4. Access to the site is controlled by a perimeter fence and a lockable gate at the entrance.
5. Proper signage is displayed at the entrance gate.
6. The facility is permitted to use four steam sterilization (autoclave) units. Currently there are two sterilization
units on site. (Photos #1 & #2)

7. No wastes were being received at the facility during the inspection.
8. No wastes were being treated at the facility during the inspection.
9. Copies of the current permit to operate and operations plan were maintained on site.
10. In accordance with the operations plan, no waste can be received or treated unless a member of management
(General Manger, Plant Manager, or Office Manager) is on site.
11. Five staff members are trained internally and authorized to treat medical waste.
12. Waste screening records were reviewed.
13. MedWaste Solutions owns two vehicles for the pickup and transport of medical waste. (Much of the medical
waste the facility receives is currently transported by Medical Waste Services, LLC in Mt. Pleasant, SC.)
14. Pathological wastes are transported by Waste Management to the Curtis Bay Energy facility in Baltimore,
Maryland for incineration. The current cost of incineration is approximately $0.52 per pound.
15. Hazardous (RCRA) wastes are transported by Southern Logistics & Environmental, LLC to the Heritage -WTI
facility in East Liverpool, OH for incineration.
16. Radiation detectors are mounted about each of the two receiving bay doors to screen for radioactive materials.
17. A "walk-in" refrigerator is used to store any waste for a maximum of thirty days, that is either not treated
immediately, or is being held pending shipment for off -site treatment.
18. Each load of waste that is received is tracked on a separate log sheet as well as on a "Waste Manifest' form,
which detail the date the waste was received, the source of the waste, the amount of waste and the type of
waste received. Waste Manifest records were reviewed.
19. Unacceptable wastes are tracked on a `By-pass Log" that also records the date the waste was received, the
source of the waste, the amount of waste and the type of waste received.
20. Once a week each autoclave is tested to ensure that sterilization is being achieved. One autoclave is tested
every Monday and the second autoclave is tested every Tuesday. These tests are logged on a "Variclave

Periodic Spore Test Log". Spore test data records were reviewed.
21. The facility is well staffed, and maintained in a well organized and clean condition.
22. Floor drains in the operational area feed into the Gastonia sanitary sewer system.
23. Emergency response is provided by the Gastonia Fire Department.
FACILITY CONCERNS & ISSUES:
24. Out of state wastes often are not required by the State of origin to meet transportation and packaging
requirements similar to those of NC.
25. Incoming wastes are often: 1) Improperly labeled; 2) Too heavy (> 50 lbs.) and; 3) Regulated medical waste
mixed with other wastes.
