PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection
STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
01 - This facility was issued a Life of Site permit on August 15, 2018 and expires on December 23, 2057. 02 - Facility has adequate signage. 03 - Facility is secured by chain link fences and locking gates.
04 - Interior roads are made with all-weather materials. 05 - The leachate drains are located at the front of this transfer station. Drains were stopped up. Need to keep free of debris to ensure the tipping floor is kept in a sanitary condition daily and to ensure no leachate is able to enter on the tarmac. 06 - Screening records were reviewed. The last screening record was dated February 28, 2019. There were load tickets in the screening record book for the month of March. Need to fill out the screening records when the load has been screened. 07 - The leachate drain at the loaded trailer storage area was also stopped up. This also needs to be checked daily. 08 - This facility has 3 Certified Transfer Station Operations Specialist: Ricky Knight, expires 6/20/2012, Melvin Brown, expires 8/15/2021 and James A. Williams, expires 6/21/2019. 09 - The service area includes the following counties: Duplin, Edgecombe, Franklin, Greene, Halifax, Johnston, Lenoir, Martin, Nash, Northampton, Pitt, Wake, Wayne, and Wilson Counties. Waste must be transported for disposal to either Sampson Co. Permit No. 82-02 or Republic Services Bertie Permit No. 08-03. 10 - The operating hours are Monday thru Friday 7:00 AM to 4:00 PM. 11 - There is starting to be an issue with windblown trash on the facility footprint. This needs to addressed and it is required to be picked up by the end of the business day and returned to the Transfer Station for proper disposal. 12 - This Facility Inspection was un-announced. There will be a follow-up UN-ANNOUNCED inspection conducted due to the issues found during this inspection. 13 – The facility operation plan was reviewed and discussed with Tyrone Sumler – Transfer Station Supervisor about issues found. 14 - Digital pictures were taken and attached to this report. 15 - Corrective action is needed in several areas of this facility from items listed above.
