STATUS OF PERMIT:
Active. The Permit to Operate issued on August 17, 2016 and shall expire March 31, 2020, according to Rule 15A NCAC 13B .0201(g). Pursuant to Rule 15A NCAC 13B .0533(a)(2), no later than October 03, 2019, the permittee must submit the Section a request for a permit review.
PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None

ADDITIONAL COMMENTS:
01 - This permit approves the continued operation of the C&DLF Unit - Phases 1 through 4, on top of the closed unlined MSWLF, as well as the onsite environmental management protection facilities as described in the
approved plans.
02 - This facility is permitted to receive permitted waste generated in the following North Carolina counties: Edgecombe, Nash, Halifax, Wilson, Pitt, and Martin. 03 - The scrap tires are being collected in an open top trailer for further processing. Area appears to be managed well.
04 - Facility Certified Landfill Operations Specialist: Travis Hudson: expires 10/03/20, Larry Moore: expires 05/24/19, Gloria A. Moseley, expires 10/21/20. 05 - The signage at this facility has all required information posted.
06 - Random screenings are being done at least 2 times per week or more. Records reviewed. 07 - The GW Monitoring wells are locked and have identification tags. 08 – Pesticide container building is getting full. During this inspection the company that grinds the containers, brought in
the grinder and will commence grinding tomorrow. 09 - This permitted facility has a Convenience Center located inside the landfill footprint. 10 - Waste at the convenience center is placed into containers and has directional signs where each type of waste goes. 11 - Edge of Waste markers are present. 12 - The C&DLF has a small working face. 13 - Cover material is stockpiled at the working face. 14 - White goods area is being managed well. 15 - Overall management of this facility is good. 16 - Digital pictures taken during this inspection.

Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
