Inc.), Jeffrey Bullard (DWM) STATUS OF PERMIT: Active EXPIRES: 12/31/2019 PURPOSE OF SITE VISIT: Adding new truck STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: Truck lettering indicating firm name, did not meet height requirements. Truck lettering indicating permit number was obstructed from view by hose on the side of tank. 15A NCAC 13B .0835 TRANSPORTATION OF SEPTAGE (b) All permitted septage management firms shall display lettering on each side of every pumper vehicle operated by the firm. The lettering shall include the firm name, town name, phone number, and septage management firm permit number as shown on the firm application. All lettering required by this Rule on the pumper vehicle shall be no less than three inches in height and legible, distinguishable from the background, and not obstructed from view. Identification shall not be removable (i.e. no magnetic signs). The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) P659509 3C7WRMCL8HG753864 1200 1) On 3/26/19 truck lettering indicating firm name, town name, phone number and permit number were permanently attached to each side of the truck were plainly visible and met height requirements. 2) On 1/4/2019 truck lettering indicating firm name and town name did not meet height requirements and the permit number on the driver side was obstructed by the hose on the side of the tank. 3) The tank was constructed of metal and appeared to be structurally sound. 4) The tank was bolted to the truck frame. 5) The septage drain valve appeared to be in working order. No septage leaks were observed. 6) All port covers and caps where in place and appeared to be in proper working order.
