Bunch (FSSS); Jeffrey Bullard (DWM) STATUS OF PERMIT: Pending EXPIRES: Pending PURPOSE OF SITE VISIT: Pre-operational STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: During the initial inspection on 1/17/2019, truck lettering indicating town name did not meet the minimum height requirement of 3 inches. 15A NCAC 13B .0835 TRANSPORTATION OF SEPTAGE (b) All permitted septage management firms shall display lettering on each side of every pumper vehicle operated by the firm. The lettering shall include the firm name, town name, phone number, and septage management firm permit number as shown on the firm application. All lettering required by this Rule on the pumper vehicle shall be no less than three inches in height and legible, distinguishable from the background, and not obstructed from view. Identification shall not be removable (i.e. no magnetic signs). The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) ZB-55358 2NKHLN9XXBM294941 3600 1) At the time of the inspection on 1/18/2019, the pumper vehicle listed above met the requirements of the Septage Management Rules. 2) On 1/17/2019, truck lettering indicating firm name, permit number and phone number, was permanently attached to each side of each truck, was plainly visible and met height requirements. The firm’s town name was permanently attached to each side of the truck, was plainly visible but did not meet height requirements. The letters were corrected and were verified to meet Septage Management Rules for height on 1/18/2019. 3) The Septage holding tank was constructed of metal and appeared to be structurally sound. 4) The tank was constructed as a dump body and attached to the truck by hinges and hinge pins welded to the truck frame. 5) The septage drain valve appeared to be in working order. No septage leaks were observed. 6) All port covers and caps where in place and appeared to be in proper working order.
