STATUS OF PAST NOTED VIOLATIONS: None
OBSERVED VIOLATIONS:
No Violations were observed. It was requested however, that corrective measures be made on tank bolts
and a tank repair (see comments on page 2) to ensure the following rule is met.
15A NCAC 13B .0835 TRANSPORTATION OF SEPTAGE
(a) Vehicles used for the transportation of septage shall be operated and maintained to prevent leaks and
spills of septage
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

(1) All lettering indicating firm name, town name, phone number, and septage management firm permit
number were permanently attached to each side of each truck, were plainly visible and met height
requirements.
(2) The tank on each truck was constructed of metal and appeared to be structurally sound.
(3) A damaged area on a tank seam on the Izuzu appeared to have the potential for leakage, the area
was repaired and reinspected on 1/24/2019.
(4) The tank on the International was securely bolted to the truck frame. Two of the bolts securing the
tank to the Izuzu frame were loose. The loose bolts were corrected and reinspected on 1/24/2019.
(5) The septage drain valve appeared to be in working order. No septage leaks were observed.
All port covers and caps where in place and appeared to be in proper working order.
