The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 – Permit to Operate was issued on April 21, 2014 and Expires on April 21, 2019. Facility has submitted a request for
permit review. 02 – Facility has signage at entrance with required information. 03 – Directional signs are present.
04 – Facility is secured by locking gates and fencing. 05 – Interior roads are constructed of all-weather materials and maintained well. 06 – Facility is permitted to receive waste generated by and in North Carolina. 07 – Certified Landfill Operations Specialist: J. William Murry, expires 5/24/2019, Archie Bush, expires 5/24/2019. 08 – Manager of Landfill Operations: Michael D. Kavanaugh, expires6/14/2021. 09 – 2018 Groundwater Monitoring Well bi-annual reports available and reviewed. 10 – 2018 Surface Water bi-annual reports available and reviewed. 11 – 2018 Landfill Gas records available and reviewed. January and February 2019 also available and reviewed. 12 – Facility has 2 flares to burn off landfill gas. Running well during this inspection. 13 – 2018 spreadsheet of waste received was available and reviewed. 14 – Incoming load scale tickets available and reviewed. 15 – ADC records available and reviewed. 16 – Cover material received records available and reviewed. 17 – Leachate System records available and reviewed.
18 – Leachate tank #A has small leak and the tank level is lower that the leak so no leachate is in the containment basin. 19 – Repairs to this tank is in the plans to be done soon. 20 – CFC removal records from white goods were available and reviewed. Last removal date was 9/7/2018.
21 – Scrap tire are taken at public convenience centers in Bertie County. 22 – Waste screening records are done at a minimal of 1% of incoming loads. 23 – Operations plan available and reviewed.
24 – Several small amounts of leachate breakouts were noted during this inspection. 25 – Facility has a Plan of Action in place to address this issue. 4 wet sumps have been already added and 1 more is scheduled to be added to pull this leachate into the leachate system. 26 – Facility has several areas with exposed waste present outside of the working face. This exposed waste needs to be covered ASAP. 27 – Windblown trash is currently being picked up by 5 people during this inspection. More attention is needed
daily to ensure compliance is meet daily. 28 – Corrective action is needed due to the items in bold on this report. 29 – Digital pictures were taken during this facility inspection. Several added to this report.
