Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Tag#JM7962, VIN#1FVACWDC46HX01612, 1500 gallons. Tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of the vehicle per the information on the application. This vehicle is in compliance at this time. 2. No septage jobs shall be performed until the firm permit is issued by the Raleigh Central office. A WWTP disposal authorization is required – please submit to the Raleigh Central office. 3. When the firm permit is issued and septage pump jobs commence then begin maintaining the required information for the truck log. 4. Enclosed is a 2019 class schedule. Ensure that you obtain the required 4 hours of continuing education before the end of the year so that the firm permit may be renewed this winter.
