Guilford County, North Carolina, File Identification No. (FID) 128652 Dear Mr. Helms:
Image First Services, LLC (IFS) operated a Medical Waste Treatment Facility (Facility), which
was located at the above-reference address to treat and process regulated and unregulated medical wastes by using a steam autoclave system under the Solid Waste Permit No. 4124-MWP-2012 [Document Identification No. (DIN) 27709]. IFS was also permitted to operate a medical waste transfer station at this Facility.
The waste operations were ceased on February 04, 2019, and the last load of treated wastes was transported to a permitted landfill for final disposal on February 06, 2019. IFS officially notified the Solid Waste Section (SWS) on February 07, 2019 that all waste operations were ceased and the processes of removing the autoclave and cleaning up the Facility (FID 1279967) was in
progress. On March 12, 2019, the SWS conducted the facility audit to inspect the completion of
the facility closure activities. According to the Facility Compliance Inspection Report (FID 1286428), the SWS determines that 1. The closure activities at the IFS facility was substantially completed.
2. The existing solid waste permit approval to operate for the IFS facility, Permit No. 4124-
MWP-2012, dated (DIN 22709) is rescinded on March 19, 2019.
3. The SWS shall start the process to release the rule-required financial assurance within a
reasonable time frame. Please contact Sarah Rice at 919-707-8287 or

Pursuant to North Carolina Solid Waste Management Rule 15A NCAC 13B. 1207(1), IFS shall maintain all records associated with the waste operations and activities at the Facility including this Closure Letter for at least three (3) years. Should you have any questions of the permitting
