The report recommending continued operation of the remediation system in accordance with the approved CAP was reviewed and approved on March 6, 2019. The paper report copies will be maintained in the MRO. The electronic copy has been loaded to the DWM's electronic document storage system (Laserfiche). The report states: • Site remediation is performed via soil vapor extraction and air sparge systems. • On December 20, 2018, Apex collected groundwater samples from twelve monitoring wells (MW-9, MW- 11, MW-12, MW-14, MW-17, MW-20, MW-21, MW-22, MW-23, MW-24, MW-25, and DMW-3). No monitoring wells exceeded the benzene 2L CAP endpoint and only MW-14, MW-17, and DMW-3 exceeded the MTBE 2L CAP endpoint. • Free product was not detected in any of the groundwater monitoring wells gauged during this phase. • Groundwater flow direction beneath the site is influenced by water table depression and mounding created by system operation at the recovery/sparge wells. Historical groundwater flow direction has been measured to the north-northwest. Petroleum compounds have never been detected in any of the area potable water supplies and sentinel wells such as MW-21, MW-22, and MW-25 continue to indicate that the gasoline plume is confined to the original source zone identified and described in both the CSA and approved CAP. Per this Notice of Regulatory Requirements: • Continue remediation system operation. • Continue semi-annual groundwater sampling/monitoring. • The next sampling event for the Monitoring Report shall be conducted in May 2019. • The next Corrective Action Monitoring and Corrective Action Performance reports are due no later than July 1, 2019. • Correct the "Prepared For:" contact information on the title page of future reports. • Future Corrective Action Monitoring reports should be submitted in accordance with the most recent revised monitoring report template "Corrective Action Performance Report" (Task 6.105/6.106) Monitoring Reports and Task 6.10516.106 Corrective Action Performance Reports. • Monitoring Reports should contain a map with water supply well locations and a table with property owner information, historic sample results tables for monitoring and water supply wells, individual contaminant isoconcentration maps, etc. as defined in Template 9A. • Corrective Action Performance Reports requirements are defined in Template 9B. • Reports failing to be submitted in accordance with the most recent template requirements will be considered incomplete. Please note that sampling and correction action must be conducted in accordance with the Guidelines for Assessment and Corrective Action for UST Releases. Failure to comply in the manner and time specified may result in the assessment of civil penalties and/or the use of the other enforcement mechanisms.
