Permit No. 1114-TRANSFER-2009 expires February 1, 2020 (DIN 22307). Operating Plan Revised October 8, 2014 (DIN 21972). PURPOSE OF SITE VISIT: Partial Facility Inspection. STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Anonymous complaint received regarding facility operations. Troy Harrison, Environmental Compliance Branch (Section), was also present during this inspection to observe the portable toilet rental operations at the property — d.b.a. Griffin Waste Services. 2. During this inspection, the transfer facility was closed and appeared inactive. 3. No unpermitted waste was observed at the facility during this inspection. 4. During this inspection, metal roll -off containers/dumpsters were stored on the northwestern side of the facility. No waste was observed in the containers (photo # 1). 5. Windblown litter was not observed at the facility. 6. Although the facility previously ceased shingle recycling operations, Handle Safe Systems, LLC, requested to retain the facility's Permit to Operate. As discussed with Mr. Griffin during the October 28, 2018, inspection, if facility operations resume, Certified Operator Training must be obtained and Handle Safe Systems, LLC, must ensure that a certified operator is on -site during transfer station operating hours. 7.
