LIST ACTIONS TAKEN:
Trash was pulled out of the working face and onto a dirt pad. It was spread out and put out by water truck. The waste in question is being left out on the dirt pad for
[he remainder of the day to make sure the fire doesn't restart and will be covered at the end of the working day.
WHAT WAS THE CAUSE OF THE FIRE:
Fhe compactor wheel hit a propane tank that was under some garbage. From the operators knowledge there was no fire prior to hitting the tank, so we are assuming
:hat when the wheel hit the tank, there was a spark that igniting surrounding garbage, fueled by the propane remaining in the tank.
DESCRIBE AREA, TYPE, AND AMOUNT OF WASTE INVOLVED:
The waste was on the MSW working face. Most of the load was cardboard, carpet and other miscellaneous bagged garbage. The total area that was affected was
loft x loft.

All of the operators are aware to be looking in dumped loads for materials that that could create a potential fire. Unfortunately, the operator did not see this propane
ank until after the incident occurred. All of the operators will be reminded again to be on the lookout for suspect materials that can be pulled out and handled in the
,orrect manner.
DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS:
For any future incident, we will continue to pull the waste out of the working face to be isolated. We will smother the fire with dirt or hit with water available from
the water truck depending on how close/available the water trick is. All waste in question will be left out of the working face for several hours to ensure that a fire
doesn't restart before being covered by the end of the day.
