The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility receives and processes wood waste and inert debris treatment and is permitted to receive land
clearing debris, untreated and unpainted wood waste, uncontaminated wood pallets and uncontaminated soil.
2. The facility's Permit to Operate and approved Operations Plan (dated April 2015) were available onsite, as well
as the current log of incoming waste and outgoing product.
3. The current log book covered the period of December 9, 2018 through February 28, 2019 and was found to
include information about hauler, truck and load type. Mr. Slater explained that he uses a system of average
weight per material type to calculate the size of both incoming and outgoing loads.
4. Mr. Slater stated that he had reduced his customer base to a minimum due to excessive contamination found in
the incoming loads. Wood waste is now accepted from only two customers. He added that he had received large
amounts of vegetative debris from the fall storms and, as a result, had been unable to keep pace with the
grinding operations. Mr. Slater further stated that he had also discontinued all incoming concrete loads because
of the size of his current stockpile. He added that adverse weather conditions have been requiring him to spend
a great deal of time maintaining the site leaving relatively little time for processing materials.
5. The facility produces fire wood, chipped wood for boiler fuel, mulch and crushed concrete.
6. A review of the 2017-18 Facility Annual Report indicated that the facility accepted 815 tons of wood waste and
70 tons of used concrete during the reporting period, and that 0.1 tons of non -recyclable waste was sent to the
Hanes Mill Landfill (permit # 34-02) for proper disposal.
Treatment & Processing Operations:
7. Windblown litter was observed to be minimal at the facility. The site appeared to be clean and well -maintained.
8. Facility access roadways are of all-weather construction and were found to be in good condition despite the
continual heavy rain received in the area over the past six months. While conditions were wet, mud did not pose
a problem for navigating the site by vehicle or on foot.
9. Signage on the gate lists the facility name, permit number, acceptable wastes and contact information, as
required.
10. The facility border is protected by fencing, berms and cement barricades, and is secured by means of a gate
which is kept locked when the facility is not in operation. The facility does not maintain standard operating
hours, and instead receives waste and dispenses product by appointment only.
11. The operations area is designed to direct stormwater runoff to a pond on the eastern side of the facility. The
pond itself was not observed during this inspection due to the wet site conditions. To contain stormwater from
the frequent rain events, a forebay had been created by grading a bowl into the center of the facility that would
store water and allow it to be pumped into the pond as the pond drains. Mr. Slater explained that the site drains
quickly, but when 5" of rainfall occurs within a 24-hour period, and this type of storm event occurs frequently,
this was the only method he'd found to be effective at preserving the site and handling the stormwater properly.
Please ensure that these stormwater control measures comply with the facility's approved erosion and
sedimentation plan.
12. Shallow puddles of standing water were observed throughout the site, caused by the excessively wet fall and
winter to date. However, no areas of ponding water were found in the waste or product stockpiles, and no
erosion was evident at the time of this inspection.
13. Land clearing and storm debris were piled near the southern end of the site, separate from the logs that are
processed to produce fire wood and lumber. A wide access road provides adequate space for facility equipment
as well as emergency vehicles to reach all areas of the stockpile.
14. Large logs are collected in the northern portion of the site. Mr. Slater pointed out the two large stockpiles of
logs on both sides of an access roadway and explained that he separates logs that are suitable for lumber that he
will sell to a sawmill, adding that the remainder will be cut into fire wood or chipped into boiler fuel.
15. Stockpiles of chipped wood waste and mulch were located around the wood waste collection and processing
area. Stockpiles were of manageable size and easily accessible for loading out material or for emergency
operations, if necessary.
16. No unacceptable materials were observed in the processed and unprocessed wood waste stockpiles.

17. Stockpiles of used concrete were staged in various locations around the site. Mr. Slater reiterated that he had
stopped accepting concrete loads so that he could process what he'd accumulated onsite.
18. Several pieces of painted concrete were observed in one of the stockpiles. Mr. Slater stated that he would ensure
that this unacceptable material is removed and disposed of properly.
19. Dust is controlled using water hoses. Water is available at several points around the site. Roadways and
operational areas can be sprayed with water when necessary. Crushing and grinding equipment is fitted with
spray nozzles so that material can be dampened as it is processed. Mr. Slater stated that, because of the wet
weather experienced over the past six months, dust has not been a concern.
