OBSERVED VIOLATIONS:
1. A significant amount of straight septage (not dewatered effluent) has been applied to the fields since the last visit.
Noted areas of heavy application (gray thick matted spray paths) that are a result of poor discharge methods –
inadequate truck speed, pressure or spreader attachment. These heavy application areas should be raked loose
to prevent crop damage. Implement changes to the discharge procedures to ensure a wide thin spray path.
Discussed situation with Eric Locklear on site who indicated that changes would be made.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Noted heavy application, thin areas of rye & scattered ruts on Fields 1, 2, 4, & 5. Repair work is needed.
The cropstand on Field 3 was in the best shape (showed the least amount of use). Recent application noted on
Field 5. No grease burns noted on any fields.
2. The required signs are posted and the access road is adequate.
3. Field markers appear accurate. It appears that the approve setbacks are being met but were not measured at this
time. No sign of land application outside the permitted boundaries.
4. Was informed that Steve Edge is now in charge of the site.
