OBSERVED VIOLATIONS:
1. .0835(b) All permitted septage management firms shall display lettering on each side of every pumper vehicle
operated by the firm. The lettering shall include the firm name, town name, phone number, and septage
management firm permit number as shown on the firm application. All lettering required by this Rule on the
pumper vehicle shall be no less than three inches in height and legible, distinguishable from the background,
and not obstructed from view. Identification shall not be removable (i.e. no magnetic signs).

Inspected pump truck tag#9183 which has been placed in Bay#1 for SDTF permit 01-04. The town name
(Burlington) on both sides of this vehicle has some letters that are less than the required three inches in
height. This was previously noted during an inspection on 8/27/2015 but was informed then that the vehicle
may be sold. As this vehicle is still in operation correct the town name on both sides of this vehicle by
March 8, 2019.

ADDITIONAL COMMENTS
1. The tank appeared structurally sound & securely attached to this vehicle (tag#9183). Outlet covers were on and
no drips/leaks were noted.
2. No other vehicles were inspected at this time and truck logs were not reviewed.
3. Enclosed is a copy of the audit from 8/27/2015 and also a 2019 class schedule.
