PURPOSE OF SITE VISIT:
Partial Inspection
STATUS OF PAST NOTED VIOLATIONS:
1. CORRECTIVE MEASURES IN PROGRESS: 15A NCAC 02C .0108(k) states: "All non -water supply wells,
including temporary wells, shall be secured with a locking well cap to ensure against unauthorized access and
use." Yadkin County was cited for failure to secure landfill gas monitoring wells with a locking well cap. Ms.
Harris stated that, to her knowledge, all landfill gas monitoring wells had been secured with locking caps during
the fall of 2018. During this inspection, time only permitted the inspection of one of the wells, MP-8, located
approximately 200' north of the scale house along the access roadway. MP-8 had been upgraded to include a
locking well cap. An additional inspection will be scheduled to verify compliance of the remaining landfill gas
wells.
2. CORRECTIVE MEASURES IN PROGRESS: 15A NCAC 02C .0108(1) states: "All non -water supply wells
shall be equipped with a steel outer well casing or flush -mount cover, set in concrete, and other measures
sufficient to protect the well from damage by normal site activities." Yadkin County was cited for failure to

equip landfill gas monitoring wells with a steel outer well casing set in concrete. Ms. Harris stated that, to her
knowledge, all landfill gas monitoring wells had been upgraded with steel casings set in concrete during the fall
of 2018. During this inspection, time only permitted the inspection of one of the wells, MP-8, located
approximately 200' north of the scale house along the access roadway. MP-8 had been upgraded to include a
steel casing set in concrete. An additional inspection will be scheduled to verify compliance of the remaining
landfill gas wells.
3. CORRECTIVE MEASURES IN PROGRESS: 15A NCAC 13B .1627(d)(1) states, in part: "Following closure
of each MSWLF unit, the owner or operator shall conduct post -closure care. Post -closure care shall be
conducted for 30 years, except as provided under Subparagraph (2) of this Paragraph, and consist of at least the
following: (A) Maintaining the integrity and effectiveness of any cap system, including making repairs to the
cover as necessary to correct the effects of settlement, subsidence, erosion, or other events, and preventing run-
on and run-off from eroding or otherwise damaging the cap system." Yadkin County was cited for failing to
maintain the integrity of the closed MSW landfill cap. No inspection of the landfill cap was undertaken during
this site visit. Ms. Harris stated that most of the required work had been completed, however the wet weather
was still posing a problem in some areas. An additional inspection will be scheduled to verify that maintenance
was performed to restore the integrity of the landfill cap.
4. CORRECTIVE MEASURES IN PROGRESS: 15A NCAC 13B .0543(e)(1) states, in part: "Following closure
of each C&DLF unit, the owner or operator shall conduct post -closure care. Post -closure care shall be
conducted for 30 years, except as provided under Subparagraph (2) of this Paragraph, and consist of at least the
following: (A) Maintaining the integrity and effectiveness of any cap system, including making repairs to the

OBSERVED VIOLATIONS:
No new violations were observed during this inspection.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS:
1. Ms. Harris stated that repairs and upgrades to the existing landfill gas monitoring wells had been completed in
October 2018. Only one well — MP-8 — was observed during this inspection due to time constraints. The well
was found to include the required components, including a permanent ID tag and a steel casing with locking lid
that was set into concrete. Ms. Harris pointed out that the casing lid had been painted bright orange to make it
more visible for equipment operators performing maintenance in the area. See the Status of Past Noted
Violations section above for a photo of MP-8 and additional information.
2. A portion of the eastern end of the closed MSW landfill was observed during this inspection. The area of the
landfill appeared to be in good condition, and the drainage ditches that direct water from the southeastern slope
to the basin located off the southeastern corner of the landfill appeared to be functioning properly. No ponding
water or signs of erosion were evident.
3. Ms. Harris stated that most of the cap maintenance had been completed, with the exception of a small area
around the toe of the landfill on the southern slope that still needs attention.
4. An additional site visit will take place to observe the remaining landfill gas monitoring wells once site
conditions make inspection safe.
