No permit required PURPOSE OF SITE VISIT: Follow up facility compliance inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - This is a Notified LCID facility. 02 - Facility has locking gate. 03 - Signage has been added. Need to add emergency contact number. 04 - DS08-010 is also located on this parcel. 05 - Access road is constructed of all-weather materials. 06 - Facility still needs to install edge of waste markers to ensure that less than 2 acres is being used. If over 2 acres, facility needs an individual permit. Side slope work is being done to move years of waste to enable staff to mark setbacks and then install markers. Wet weather has slowed this process. But a lot has been done. 07 - Facility will have 3:1 Side slopes when work finished. 08 - Non-conforming waste has been removed from past years and now being removed when any is brought to this facility and taken to permitted facility that excepts these types of waste. 09 - Matt Wilson was called to this un-announced facility inspection when John College arrived at the facility. 10 - The new management of this facility is taking the proper steps to ensure compliance is met. 11 - Soil cover is being done as side slope work continues. 12 - Digital pictures were taken during this inspection. Several were added to this report.
