Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1- Inspected replacement vehicle, no tag yet, VIN 2NP3LJ0XXKM620490, 4000 gallons. This firm will only run on truck – the 2019 firm permit has been issued. 2- The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of the vehicle. This vehicle is in compliance at this time. 3- Discussed the required information to be maintained for the truck logs. Ensure that the gallons for each pump job is listed (particularly in regards to the new vehicle with a 4,000 gallon capacity) and also each specific discharge location. Enclosed is an example copy of the truck pumping log. 4- Mr. Kirby attended the Hickory training location in January.
