We have ceased waste operations at our facility and our desire is to close the medical waste treatment facility.

Subject: Image First Services LLC 41-24 Med Waste Treatment Facility to Cease Operation (Spring 2019)

Dear Mr. Helms:

I am informed that your facility has an intention to cease waste operations and to close the medical waste
treatment facility. Upon the final decision of ceasing waste operations at the facility is made, please send a
written notification of site closure to the Solid Waste Section (SWS).

A follow-up inspection and confirmation procedures will be conducted by the SWS representatives
accordingly. If you require a specific close-out document please submit the specific request terms in writing;
otherwise, the SWS may produce a standard close-out document after the site closure is deemed completed.

Deb,

Please inform the ESS responsible for Guilford County that 41-24 plans to cease operation and close the permit in ~
March 2019.
I informed Billy Helms (see contact info on attached insp. report) that SWS-FOB will conduct an inspection when the
facility is ready to close in the spring. I also told him that, upon his request to the Permit Branch and inspection from
FOB verifying that the facility has ceased operation and contains no solid waste, that the permit can then be closed-out.
